E' la storia di due amici d'infanzia di Corleone, Michele e Vito, uno dei quali diventa sindacalista, l'altro mafioso. Il film inizia tra le lotte di terre negli anni cinquanta e 60 in Sicilia. A Vito Gargano (Giuliano Gemma) su ordine di Giusto Provenzano, "uomo d'onore" locale (Francisco Rabal), viene chiesto di uccidere Michele Labruzzo (Michele Placido) che si oppone ai soprusi di Provenzano. Alla fine Vito uccide Michele e diventa uomo di fiducia di Provenzano per poi uccidere anche quest'ultimo e prenderne il posto.
